I agree with the injury system proposed by Fleshhound.

I have some ideas when it comes to disease treatment, when diseases become a thing. Could be anything from food poisoning to serious sepsis (the latter being linked infections that are untreated and get out of hand to wounds that get infected)

Diseases could have stages too, which increases complexity, costs and difficulty of treatment but also making it easier to diagnose.

In addition, in diagnostic medicine, we have signs and symptoms. Symptoms are things experienced by the patient and signs are things observed/heard by the physician.

When a patient has a disease or a metabolic disorder (as opposed to injuries) let’s take the example of carbon monoxide poisioning, the symptoms the patient (or the person who brought them in) reports would be difficulty in breathing and blurry vision. They have to report this to the physician. Based on this, it could be anything from poison gas to infection, or a lung condition. The Doctor then looks for signs and RPs further asking the patient about what they might have been doing. He learns that the patient was an electrical engineer operating a fossil fuel generator in a mine, and also observes that parts of the patient’s skin has a cherry red hue (sign) and so deduces that the patient might have Carbon Monoxide poisoning, and so treats them with 100% Oxygen.

TL;DR- For conditions other than injuries, let’s have the sign-symptom system where the patient experiences symptoms that they have to report to the Doctor, the Doctor observes signs and questions the patient on his disease history and then from both symptoms reported and signs observed, the doctor diagnoses what is wrong and treats. Therefore, I believe personally, this fosters a need for good bedside manner on part of the doctor to make the patient feel safe opening up and a need for the patient to trust their doctor, driving RP.
